• The sworn disclosure must identify the insurer, each insured, liability limits, known policy or coverage defenses, and include a copy of the policy.
• The disclosure applies to each known liability policy, including excess or umbrella insurance.
• The insured or insurance agent, on written request, must disclose the name and coverage of each known insurer and forward the request to affected insurers.
• The insurer must amend its statement immediately upon discovering facts requiring an amendment.
